Job Description:

Under the direction of the Director of Healthy Living and Strategic Partnerships, the Director, Community Integrated Health is primarily responsible for community, evidenced-based health interventions. Specifically the Director, Community Integrated Health will be responsible for recruitment and implementation of the YMCA of Delaware established signature healthy living programming (may include established programs such as the YMCA’s Diabetes Prevention Program, Healthy Weight and Your Child and LIVESTRONG at the YMCA after program launch phase). This includes but is not limited to providing support and guidance to medical partners, engagement with those providers to set up direct referrals to all healthy living programs, working with established employers and direct payors on program delivery, as well as ensuring that each program is meeting fidelity standards, and is achieving quality performance outcomes.

This position will work with health seekers, employers, community leaders, YMCA staff & leadership and representatives from the medical and insurance community. The incumbent must have a passion for service and improving the health of our community. They must also possess the ability to work with data, be detail orientated and have exceptional organizational and communication skills. This position must perform program responsibilities in accordance with the commitments made to funders, partner organizations and the policies, goals, values, and mission established by the YMCA of Delaware.

Qualifications

• This position requires a BA/BS degree in Public Health, Wellness, Exercise Science, Health Promotion, Health/Physical Education, Nutrition or a related field, or its equivalent in experience, and 3-4 years experience in YMCA/similar agency and a strong commitment to the YMCA philosophy and values.
• Additional education/certification and or related experience in the area of adapted programming and a national certification (ACSM, NSCA, or ACE) a plus. Must be able to stay current with trends in the field.
• Must have skills in one-on-one coaching/assessments, group facilitation, consensus-building and communication. The position requires continual pursuit of further education in the areas of program facilitation, implementation, supervision, and member service.
• Must have or the ability to obtain required training and certification, including but not limited to internal YMCA trainings in YMCA’s Diabetes Prevention Program Instructor/ Healthy Weight and Your Child Program Instructor and/ or LIVESTRONG at the YMCA Instructor
• Current YMCA of Delaware work experience and systems knowledge a plus.

Essential Functions

• Serve effectively as a team contributor on all assignments.
• Provide support to healthy living staff, program coaches, and members including the coordinator of integrated health.
• Lead & assist with the clinical outreach, finding outreach opportunities, and defining new potential partnerships for community engagement. Pursue further clinical integration of YMCA evidenced based-signature programs into care process models throughout our region.
• Share responsibility for the retention and growth of the programs and initiatives.
• Share responsibilities for program supervision, public relations, planning and promotions as agreed.
• Ensure delivery of the signature healthy living programs in a manner that complies with and or exceeds published standards of the CDC, Y-USA and other applicable organizations.
• Work with branch staff and community partners to ensure that all resources necessary to achieving program goals are available. This includes but is not limited to intake space, classroom space, work station and relevant materials that facilitate compliance with HIPAA regulations, comprehensive class schedule and provider communication system.
• Serve as a liaison between the YMCA and community partners within our entire service area. Paying special attention to medical providers, health departments and like-minded community based organizations.
• Align program sales and development strategies with those of the Marketing and Communications and Membership Departments. Aid with communications, promotions, social media and educational materials.
• Work collaboratively with the membership department to ensure member satisfaction, conversion and retention.
• Ensure program fidelity and implementation by monitoring and assessing the results of our programs.
• Understand and enforce all program standards and procedures at all times.
• Share responsibility in managing programs so that they operate within established fiscal boundaries.
• Attend and participate in all meetings, program development activities, YMCA events and healthy living program initiatives.
