Hartford HealthCare Rehabilitation Network is the premier rehabilitation provider in Connecticut. We provide rehabilitation services across the continuum; physical therapy, occupational therapy, speech language pathology and athletic training services at our 5 acute care hospitals, short-term rehabilitation facilities, assisted living facilities, homecare, and 30+ outpatient centers. We are opening a 26-bed inpatient rehab unit at Hartford Hospital in the 2018. Overview
We're seeking a Clinical Rehab Program Manager (Physical Therapist, PT) to lead our Spine program.

Position Description
The Spine Clinical Program Manager provides direct patient care and in addition:
Provides strategic direction and innovative ideas for the development, education, implementation, and evaluation of evidence based practice models and clinical guidelines for spine care across the HHC enterprise.
Reduces unwanted variation by utilizing key performance metrics and current research to develop and implement these standards of care.
Collaborates with rehab leadership to establish meaningful programmatic goals that align with divisional and system priorities to grow existing spine services by ensuring evidence based practices to meet market demands.
Effectively communicates these goals and objectives to the programs staff and monitors progress during the year.
Demonstrates an understanding of and accountability for key performance metrics, process measures and outcome data and the ability to monitor and develop appropriate action plans specific to the program.
Coaches and mentors regional program providers and ensures that program staff meet or exceed an established standard of excellence, including efficiencies, patient experience and clinical outcomes.

Qualifications

Minimum Requirements
Graduate of an accredited PT program (MS/Doctorate)
CT PT licensure or eligible
Minimum of 5 years experience treating individuals acute, chronic, and post-operative spine conditions
Clinical Specialty certification preferred
Strong verbal, written and interpersonal skills
Travel across HHC service area

Knowledge, Skills and Ability Requirements
Demonstrated ability and willingness to teach on a local, regional and national level
Demonstrated experience collaborating with key stakeholders including physician referral partners
Ability to understand and analyze financial/operational metrics
Ability to develop and implement action plans to improve clinical outcomes
Computer proficiency in Microsoft Word, Excel, PowerPoint as well as electronic heath records and other software applications
