POSITION SUMMARY :
Provides state of the art nutrition assessment, Medical Nutrition Therapy (MNT) in a variety of settings, to patients and their families/care-partners, helping to optimize their health needs. Participates in the development of education curricula and materials by writing original material and/or reviewing the work of others. Has responsibility for the assessment, planning, implementation and evaluation of the patient centered health and wellness program. Collaborates with others in identifying the needs, developing and providing clinical programs and services. Promotes and disseminates quality nutrition and wellness information to professionals and the public following established Clinic Division guidelines. Participates in case management and patient care plans and goal setting.
POSITION EDUCATION/QUALIFICATIONS :
College degree in Nutrition or Dietetics required
Registered with the American Dietetic Association, State Licensure, required or must sit for the exam within six months of eligibility.
Computer skills required with knowledge of Microsoft Office suite
Excellent written, verbal and interpersonal communication skills required
Some evening or weekend work may be required
Bilingual English/Spanish preferred
Position requires travel, valid drivers license and vehicle insurance are required
JOB KNOWLEDGE/EXPERIENCE :
At least 1 1/2 years experience in an MD office or three (3) years of education/counseling (with chronic diseases).
Knowledge of State, Federal, and JCAHO regulations
High degree of competency/experience in general. Able to respond to patient needs by interpreting facial expressions and/or verbal communications
Requires good organizational skills, reasoning ability and good independent judgment and working with frequent interruptions
Skill in nutritional principles through appropriate assessment of status/needs and development of care plan based on appropriate nutritional adequacy through functional medicine assessment
Skill in patient/staff education by effectively developing and conducting educations programs in an enthusiastic, motivational manner with a professional manner and image, in front of groups and serving as a resource within the department
Skill in participating effectively as a member of interdisciplinary team, interacting appropriately with physicians, nurses, and other health educators
Ability to communicate and work effectively with a variety of patients, families, and staff, to help patients problem-solving about making appropriate lifestyle changes
Responsibilities:
RESPONSIBILITES:
Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices
Work directly with the physician to develop and implement clinical/education programs that service patient needs. This includes determining market and patient expectations/needs and developing programs collaboratively with others
Assess patient nutritional status, diet history, and food habits. Helps determine care plan for each patient
Attend appropriate practice committee meetings to remain involved and active member of clinical and educational staff. Actively participate in patient case management and goal setting.
Participates in quality control/quality improvement activities within ones scope of practice that promotes quality service.
Participates in the development, updating and revision of educational materials, and participates in educational seminars and support groups as needed.
Provides ongoing nutritional counseling regarding implementation of weight loss or other strategy as prescribed by the physician
Provide state of the art nutrition assessment and education. Applies accepted standards of care and education adopted by the practice as indicated to patients and their families/care-partners to help optimize nutritional management.
Maintains appropriate and accurate documentation in EMR, meeting standards and providing needed information for clinic and department evaluation and actions. Demonstrates ability to incorporate current research and practice into educational materials, protocols, policy and procedures, and participates in research projects as required/requested.
Evaluate and assess patients management and education needs, self care behaviors, obstacles to successful implementation of recommendations in a variety of settings.
Develop individualized education and management plans to include appropriate follow-up.
Provide management consultation in person and over the phone to patients
Coordinate consultations and referrals and facilitate communication between patient and providers as needed.
Measure and documents through written tests and/or verbal demonstration, patients understanding of information taught.
Coordinates and participates in case management, including community referrals and establishes good rapport and cooperative working relationships with hospital personnel.
Maintains professional registration and certification and state licensure
Stays up to date on current thought and research in functional medicine strategies by attending conferences, reading relevant literature, and using Internet resources as available ensuring that the practice Education Services as well as ones professional services are consistent with the provision of state-of-the-art care. Remains current in teaching learning principles and utilizes a variety of education delivery options.

B. Working Conditions : The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
