Nutritionist Aide Who we are La Clnica de La Raza is a community based health center committed to providing culturally appropriate high quality and accessible health care to the diverse communities of the San Francisco Bay Area We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care As health care activists we are dedicated to ensuring that individuals who do not have healthcare coverage receive the same quality healthcare as those who do From our genesis as a community health center in 1971 we understood that we cannot heal our communities without also addressing the economic and social factors that affect health Many times this requires that we go outside and provide services in other settings that link individuals to our health center for preventive and primary care Over 90000 individuals come to La Clnica because it is a welcoming place that addresses the whole person coordinating and connecting them to a broad network of services to improve and maintain their health and well being While we are still known for our activism and spirit of social justice we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda Contra Costa and Solano counties Who you are Under general direction the individual in this position is responsible for conducting nutrition screening assessment and counseling as well as implementing nutrition care plans and breastfeeding support for our patients Major Areas of Responsibility include but are not limited to Conducts nutrition screening assessment and counseling and implements nutrition care plans for all but high risk patientsAssures appropriate records are updated ie records nutrition assessment dates follows up on nutrition assessment appointments with Nutritionist II obtains appropriate signatures on re interview forms during appointmentsMaintains documentation by charting dietary intake and recording recommendation given to clients in family folders and medical recordsTeaches group classesAssists with the development translation and evaluation of culturally relevant nutrition education materialsAttends weekly staff meetings and other workshops as requiredOther duties as assigned by WIC supervisorMinimum Job Requirements Requires completion or intent to complete CPA Competent Professional Authority certification as mandated by the state of California; minimum one year of individual nutrition counseling experience preferredMust hold a current BLS Basic Life Support Certificate You will have 30 day grace period to obtain your certificate from the date of hireKnowledge of contemporary principles and practices of nutrition; interpretation and implementation of nutrition guidelines for infant children and pregnant and breastfeeding and non breastfeeding women and other patientsKnowledge of nutritional value of foods; meal planning and preparation that is culturally appropriate; food safetyKnowledge of principles methods and materials used in nutrition counseling education and screeningKnowledge of Motivational Interviewing techniques preferredAbility to work independently as well as collectively on projectsStrong interpersonal verbal and written communication skillsAbility to work across cultures and demonstrate support of diversity equity and inclusionBilingualSpanish required
